Asked on #mantishelp, got the following in response:

(8:59:14 PM) portej05: hi, I've got a question about mantis, but I'm not an administrator nor do I have access to the configuration of the current setup
(8:59:42 PM) portej05: We're running mantis 1.0.5 (fairly old)
(8:59:57 PM) portej05: Thing is, it's sending out emails for everything, except account activation
(9:00:05 PM) [KK]Kirill: sorry, I forgot this version
(9:04:15 PM) nuclear_eclipse: portej05: my only recommendation is to upgrade to 1.1.7 (latest stable), which should not only fix that, but also fixes many security issues that were present in 1.0 and early 1.1
(9:06:03 PM) portej05: Cheers nuclear, I'll pass that on
(9:06:17 PM) portej05: Is the upgrade process relatively painless?
(9:07:19 PM) nuclear_eclipse: assuming you don't have any modifications to the code (only changes to custom_* and config_inc), then it should be as simple as extracting the new code over top the old, and visiting /admin/install.php to run the upgrade procedure
(9:09:04 PM) thraxisp left the room (quit: "Leaving.").
(9:09:17 PM) portej05: Thanks!

You do know the possibility exists that it's not Mantis that was ever the issue with the email. It's possible the mail server at scp.indiegames.us is out dated as well and is sending out email without proper return routing info in the mail headers so is being bounced as spam by any servers it routes through.

Just so you can see what I mean I went through some old Mantis emails:

Most recent when it was still using a game-warden email and worked for some people:

If you look it's originating from serv1.maxgaming.net but says it's from game-warden. Servers may toss that due to a bad return path.

Now an even older one when it didn't work for hardly anyone:

Again mgo1.maxgaming.net but sent from fs.scp@verizon.net

Now one from HLP that does work:

You can see that the from and return are both hlp.staff@gmail.com and the server and return are hard-light.net and game-warden.com.

So my theory is that the problem is somewhere with anything originating from or going through maxgaming.net
